  1. After the download, you will receive an email with a link to the survey up to 14 days after the download completed.
  2. Fill out the survey. Make sure your shirt size is correct.
  3. Click SUBMIT when done.

How to Win Prizes

1. After you’ve received your shirt in the mail, you might find a GOLDEN WINNER ticket inside the package.
2. If found, you’re one of the 5 lucky winners to win a prize! Prizes are listed in the Rules & Terms section below.
3. If not found, you still have a chance to win the GRAND prize.

How to Win the GRAND Prize

1. Post a photo of yourself wearing the shirt on Facebook and/or Instagram.
2. The photo caption must include the hashtag #IHeartZ.
3. IMPORTANT: Make sure the photo is public when posting, so the judges are able to view your photo.

RULES & TERMS

NO PURCHASES OR VIDEO RELAY SERVICE CALLS NECESSARY. A PURCHASE OR VIDEO RELAY SERVICE CALL DOES NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED.

1. Eligibility. This Sweepstakes is open to deaf and hard of hearing legal residents of the 50 United States and the District of Columbia, who are 18 years of age or older at time of entry and who have Internet, email address, and no Z5 number as of June 30, 2015. Employees, contractors and subcontractors of ZVRS ("CSDVRS, LLC ") and its affiliates and agents and their immediate family members and persons living in the same household with them are not eligible to enter or win.
2. How To Enter. You may enter the Sweepstakes anytime between July 1, 2015 through September 30, 2015 by downloading Z5 Desktop and/or Z5 Mobile, verifying the new Z5 Number, and completing the Z5 Survey sent via e-mail up to 14 days after the download completed.
When you complete the Z5 Survey, you will receive a Z t-shirt. Upon the t-shirt’s arrival, you might find a Golden Ticket inside the package. A prize will be rewarded to each Golden Ticket Winner that has made contact with ZVRS via email at media@zvrs.com or VP at Customer Care 866.932.7891. Limit: one Golden Ticket per person and per email address and per Z5 phone number. A total of 5 Golden Tickets will be packaged with t-shirts throughout the Sweepstakes period. You may have a chance to enter a drawing for the Grand Prize. To enter, you must do the following actions:

• Post a photo of yourself wearing the ZVRS t-shirt on Facebook and/or Instagram.
• The caption for your photo must include the hashtag #IHeartZ along with your comment.
• Make sure the photo is public when posting, so the judges are able to view your photo.
  
  Use of a false email account will disqualify an entry. CSDVRS, LLC is not responsible for technical, hardware or software malfunctions, lost or unavailable network connections, or failed, incorrect, inaccurate, incomplete, lost, late, misdirected, garbled or delayed entries or other communications or other technical problems related to the Sweepstakes or Sweepstakes entries. The use of scripts or any automated system to enter is strictly prohibited. CSDVRS, LLC, in its sole discretion, reserves the right to disqualify any person who tampers with the entry process or the operation of the Site, or who otherwise violates these rules.
  
  

3. Prizes. 5 Golden Ticket Winners will choose and receive one of the following prizes: 42mm Apple Watch, Apple iPad mini 3 16 GB, Samsung Galaxy Tablet 8.4’’ 16GB, or Motorola Moto 360. Each prize is valued up to $400. The Grand Prize is roundtrip airfare to Orlando, Florida for 4 people, 2-day Walt Disney World® park hopper passes for 4 people and a 3-night stay at a Walt Disney World® Resort Hotel.

Odds of winning prize depend on the number of t-shirts shipped out. Prize is not transferable and subject to availability. No substitution of prize for cash or other goods and services is permitted, except that CSDVRS, LLC reserves the right to provide a substitute prize of approximately equal value. All expenses not specified above, including, without limitation, all federal, state, provincial, and local taxes, and international tariffs, are the sole responsibility of the individual winner.

4. Drawing. Grand Prize potential winner will be determined by random drawing on or about Tuesday, October 14, 2015 from all eligible photo entries received thru the public search of the hashtag #IHeartZ on Facebook and/or Instagram. The drawing will be conducted by CSDVRS, LLC or its designee, the judge of the Sweepstakes, whose decisions are final and binding. Potential winner will be notified on or within 10 days of drawing by an email sent to the email address specified at the time of entry. If the potential winner does not respond within 14 days of the date of the prize notification, fails to comply with the instructions contained in the prize notification within that time period, is ineligible for the prize, or refuses to accept the prize,CSDVRS, LLC reserves the right to randomly choose an alternate potential winner from the pool of remaining eligible entries. Return of any prize notification as undeliverable will result in disqualification and selection of an alternate potential winner. In the event of a dispute concerning the identity of the person submitting an entry online, the entry will be deemed to be submitted by the person in whose name the email account for the email address provided to CSDVRS, LLC is registered. Potential winner may be required to provide a driver’s license and/or other evidence of identity in connection with claiming the prize. Winner may be required to complete and sign an Affidavit of Eligibility and Liability Release and, except where prohibited, a Publicity Release in the form provided by CSDVRS, LLC, within fourteen days of attempted notification in order to claim prize. If a minor wins a prize, it will be awarded in the name of the minor’s parent or legal guardian, who will also be responsible for signing any necessary releases, consents and affidavits. An entrant is not a winner unless and until the entrant’s eligibility and has been verified and entrant has complied with all Sweepstakes requirements. Prize will be awarded. Allow six weeks for delivery of prize.
5. General Conditions. CSDVRS, LLC has the right to cancel, terminate or modify this Sweepstakes if it cannot be completed as planned due to computer viruses, bugs, tampering, unauthorized intervention, technical failures, or other conditions beyond CSDVRS, LLC’s control, and to select winner from eligible entries received on or before the termination date. CSDVRS, LLC is not responsible for any failure to contact entrants, whether due to technical or human error. By entering this Sweepstakes, entrants agree (i) to be bound by these rules; (ii) that any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this Sweepstakes but in no event attorneys’ fees; and entrant hereby waives all rights to claim punitive, incidental, and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased, SOME JURISDICTIONS DO NOT ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES, SO THE ABOVE MAY NOT APPLY TO YOU; (iii) that CSDVRS, LLC has the right to use entrant’s first name, last name, and city and state on CSDVRS, LLC’s website and in any and all publicity and advertising CSDVRS, LLC without any further attribution, notification or compensation to entrants except where prohibited by law; (iv) to be contacted by CSDVRS, LLC by telephone, mail or email regarding this Sweepstakes; and (v) to release and hold harmless CSDVRS, LLC and its agents for any liability, damages, loss or injury resulting from entrants’ participation in this Sweepstakes or the acceptance, possession, shipping and handling, loss, use or misuse of any prize awarded in this Sweepstakes. Entrants and winner assume sole liability for injuries, including personal injuries and/or damage to person or property, caused or claimed to be caused by participating in this Sweepstakes or the acceptance, possession, shipping and handling, loss, use or misuse of any prize awarded. The Sweepstakes is governed by the laws of the United States and all claims must be resolved in the United States.
